Maximizing Tumor Resection With High-Field MRI

The use of intraoperative MRI (IoMRI) in glioma surgery has been shown to improve overall and progression-free survival (OS and PFS), increase the extent of resection (EOR), and aid in decision-making for residual masses, ultimately enhancing the quality of care for patients with brain cancer.

Leyla Mirzayeva and the team aimed to assess Io MRI’s influence on OS, PFS, EOR in glioma patients and its role in surgical decision-making.

The study included 153 glioma patients who underwent surgical treatment from 2013 to 2023, with 125 receiving Io MRI guidance. The remaining 28 formed the control group without Io MRI. Patient demographics, initial radiological diagnosis, tumor characteristics, EOR, histopathological findings, and follow-up durations were documented.

The results showed that in cases where Io MRI was employed, the rate of tumor recurrence was significantly lower compared to the control group (P< .0001). Additionally, in 45 Io MRI cases, surgery continued, leading to a rise in the gross total resection (GTR) rate from 33.6% to 49.6%. Notably, patients initially diagnosed with low-grade glioma had a higher frequency of GTR compared to those with high-grade glioma. Furthermore, occipital gliomas displayed the shortest OS.

The study conclusively demonstrated the efficacy of high-field MRI in reducing tumor burden, enhancing PFS, and enabling maximal resection during initial surgery.
