MRI or Expert Ultrasound In Preoperative Staging For Early-Stage Cervical Cancer

The SENTIX study evaluated the use of sentinel lymph node (SLN) biopsy without pelvic lymph node dissection (PLND) in the treatment of patients with early-stage cervical cancer. Researchers reported the sensitivity of magnetic resonance imaging (MRI) and expert ultrasound (EUS) in preoperative clinical staging.

This study involved 47 research locations across 18 countries and focused on patients with cervical cancer stages ranging from 1A1/LVSI+ to 1B2, according to the FIGO 2018 classification. Enrollment occurred between May 2016 and October 2020, and either pelvic MRI or expert ultrasound was used for preoperative local staging, based on the investigator’s choice.

Findings from the 690 participants indicated that MRI was used in 46.7% of cases, expert ultrasound in 43.1%, and both imaging methods were used in 10.1%. Unforeseen parametrial involvement was observed in 26 patients (3.8%) during pathological examination, while sentinel lymph node metastasis was detected in 68 patients (9.9%). In these metastatic cases, 54.4% involved micrometastasis and 45.6% macrometastasis. MRI and EUS demonstrated similar capabilities in assessing tumor size and failing to identify parametrial or macrometastatic LN involvement. Using both imaging methods together did not improve the outcome.

Pelvic MRI and EUS have comparable effectiveness in evaluating critical clinical factors for pre-surgical staging in early-stage cervical cancer.
